Someone asks whether a deliverable was checked and against what. Answering means finding the reviewer, trusting their recollection, and hoping the evidence still exists somewhere. It usually does, spread across a mailbox, a spreadsheet and a folder.
The work was almost certainly done. Proving it is the part that costs weeks.
Review quality is managed by the discipline that performs it. Review assurance is owned by someone who was not in the room.

A low finding count means one of two things, and they are opposites. Either the deliverables are clean, or most of them were never checked. Coverage is what tells you which.
Every document ingested into a project is either included in a run or it is not, and that is visible rather than assumed.
